TPO rejected claim/line because payer name is missing. (Use status code 21 and status code 125 with entity code )

The primary payer carrier code needs to be sent in the 'Secondary Claim Info' section where the payer ID usually goes.

If that info is already there, resubmit the claim as we have updated our system on 7/3/12 to allow codes of 2 digits or more to be sent. If the payer ID is there, please refer to the payer for the correct carrier code.
