What happened
In box 21, there's a gap between 2 of the diagnosis codes.
Resolution
Need to move the diagnosis codes so there are no gaps between the codes. If no gaps are present, please contact Customer Service via live chat, email ([email protected]) or phone (+1 727-688-7008 option 1).
